Lifespan Opportunities in Washington and Hancock Counties

Family Coaching Program

DCP's family coaches are dedicated to joining with families and individuals in an effort to provide support, and assist in finding resources as they work toward family, personal, and occupational goals. Services are tailored to the family, or individual, and their personal goals and needs.

Once a referral is received, the coach sets up a meeting with the potential participant. These visits can be at our offices, at the participant's home, or at another appropriate meeting spot convenient for the individual. The first meeting is designed to get to know each other and for the advocate to learn more about the changes the family/individual wishes to make. Following that initial meeting, the coach will work with the participant to set goals and establish steps to achieve those goals; identify appropriate resources; and support the family/individual as they move along the path to achieving their goals and dreams. That support is customized to individual needs but always includes regular meetings to measure progress, adjust plans, and identify resources.

Possible goals can include changes in employment, housing, healthcare, finances, parenting, education, transportation, community involvement, daily living skills, or social growth.
